PCMH Case Study: Focus on Pharmacy Innovation

Little Rock Family Practice Clinic is a 12-physician practice serving approximately 31,000 patients through two locations.  The clinic is one of the original 69 Arkansas clinics originally chosen to participate in the Center for Medicare and Medicaid Innovation's Comprehensive Primary Care Initiative.  Recognizing that common chronic issues across its panel often require medication intervention and adherence, the clinic hired a pharmacist expert in drug effectiveness and interactions to  achieve better health outcomes and avoid costly hospital stays.  Read more.
